OPEN POSITION: Transitions Coordinator, Tri-County Independent Living (Eureka, CA)

This role focuses on identifying needs of individuals who are transitioning, or
desire to transition, from institutional care to more independent living . You will
provide support in helping them make their own choices and live in a fully
accessible, integrated community or return to their own homes. Bachelor’s degree in social services related field or 2+ years of related work, experience working with people with disabilities and/or older adults preferably in an Independent Living Center or Aging and Adult services organization.

Compensation: $19.00 -$22.00/hour depending on experience, competitive benefits and flexible scheduling.
